ReportING information to CPI

There are a few things we will need from you.

  • New Employee Notification

When you add a new employee who will be eligible for benefits, notify both your carrier and CPI of the new addition. Upon employment, a COBRA initial notification will be provided to all new employees and their dependents. This is required by Federal Law.

  • Qualifying Event Notification

You are required to notify CPI within 30 days of any of the following events:

    • An employee is terminated, fired, resigns, quits or terminates the employment relationship with your company in any way
    • An employee has a reduction of hours to the extent that they no longer meet the requirements of the plan
    • Death of an employee
    • Employee/Retiree becomes entitled to Medicare, causing dependents to lose coverage
    • Employer Bankruptcy
  • Carrier/Rate Changes

You are required to notify CPI of any changes with your plan, including a change in carriers, eligibility requirements, and rates. The Carrier/Rate Information Form can be used to notify CPI of any such changes.

  • Employee Divorce or Legal Separation

Employees or their beneficiaries must notify you in the event of a divorce or legal separation. In turn, you must notify CPI within 30 days of receiving notice.

  • Employee's Dependent Child Ceases to be a Dependent

Employees or their beneficiaries must notify you with any change in dependents. In turn, you must notify CPI within 30 days of receiving notice.

COBRA regulations provide a maximum of 30 days for you to notify CPI of any one of these qualifying events. It is strongly encouraged that you notify CPI as quickly as possible.
